OhioHealth Group Credentialing Services has implemented the new credentialing forms, processes and timelines that resulted from the passage of Ohio’s Health Care Simplification Act (House Bill 125) in early 2008. As a result of this change, OhioHealth Group Credentialing Services is participating with the Council of Affordable Quality Healthcare (CAQH) Universal Credentialing Database initiative for the following entities:
- OhioHealth Group (HealthReach / HealthReach Preferred products)
- The Medical Group of Ohio
- Riverside Methodist Hospital
- Grant Medical Center
- Doctors Hospital
- Dublin Methodist Hospital
- Grady Memorial Hospital
- The American Kidney Stone Management Ltd.
CAQH is an online service where practitioners can provide standardized credentialing information to multiple organizations without filling out multiple forms. Simply enter your information into the data base and authorize OhioHealth Group Credentialing Services to access your information. For your convenience you may download a copy of the CAQH User Guide under the “Forms” section of this website for questions related to completing your application on the CAQH website. Upon completion of your CAQH application download the CVO Application on the left side of this webpage under “Forms” to obtain additional doucmentation that is necessary for the credentialing of your application. The CVO Application Packet will contain the following:
- Hospital specific documents if you are applying to any of the 5 Central Ohio OhioHealth hospitals,
- Provider Agreements if you are interested in participation with The Medical Group of Ohio or OhioHealth Group (HealthReach / HealthReach Preferred products).
